Save Time at Your First Appointment
To make your first visit as smooth and efficient as possible, we invite you to download, print, and complete our new patient forms before you arrive. Bringing completed forms to your appointment allows us to spend more time focused on your care rather than paperwork.
If you prefer, you are also welcome to arrive 10–15 minutes early and fill out the forms in our office. Our friendly front desk team will be happy to assist you with any questions.
Please note: Our forms are provided as a courtesy for your convenience. If you have trouble downloading or printing them, simply arrive 10–15 minutes before your scheduled appointment time and we will provide paper copies in the office.

Complete this form with your personal information, contact details, and emergency contact before your first visit.
Medical & Dental History Form
Help us provide the safest, most personalized care by sharing your current medications, allergies, and past dental and medical history.
Dental Insurance Information Form
Provide your insurance carrier, policy number, and subscriber information so we can verify your benefits before your appointment.
HIPAA Privacy Notice & Acknowledgment
Review our privacy practices and sign to acknowledge receipt of our Notice of Privacy Practices as required by federal law.
Financial Policy Agreement
Review and sign our financial policy, which outlines payment expectations, insurance filing procedures, and cancellation policies.
Instructions for New Patients
- 1Download and print all applicable forms listed above.
- 2Complete each form thoroughly and legibly in ink.
- 3Bring the completed forms along with your photo ID and insurance card to your appointment.
- 4Arrive 5–10 minutes early to allow time for check-in.
- 5If you have recent dental X-rays from a previous provider, please bring those as well.
